To: Jennifer Lynn Smith and Michael Patrick Macey, Jr. a/k/a Patrick Macey, parents of Michael Patrick Macey, III, a minor born on May 9, 2007 in Allegheny County, Pennsylvania, Miley Ann Macey, a minor born on April 2, 2010 in Allegheny County, Pennsylvania, and Myranda Marie Macey, a minor born on May 8, 2012 in Allegheny County, Pennsylvania.
